AFHP collects personally identifiable information when you contact us in any way and we also receive personally identifiable information from our business partners.
Once you send your e-mail(s) or other communications to AFHP, or establish a web site Log-In, you are not anonymous to us.
AFHP uses your information for four (4) general purposes: (1) to fulfill your requests for certain information and services; (2) to contact you about our services and/or related services (3) to ensure the security of the Site and (4) to share your information with our employees and affiliates to determine if we can or will be able to enter into a business relationship with you.

AFHP will not sell or rent your personally identifiable information to anyone other than in furtherance of a change in control of AFHP.

SECURITY AND INTEGRITY OF INFORMATION
We treat our data as assets that must be protected against loss and unauthorized access. We employ reasonable information security techniques to reasonably protect confidential information from unauthorized access by users including generally accepted information security techniques such as firewalls and access control procedures (e.g. password protection). Intentional access to a user’s personal information is limited to those whom the user has authorized access and those AFHP employees who have a legitimate business need for that information. Notwithstanding the foregoing, no security system is absolute. AFHP CANNOT AND DOES NOT WARRANT THAT YOUR PERSONAL INFORMATION WILL BE PROTECTED AGAINST LOSS AND/OR UNAUTHORIZED ACCESS.
RETENTION OF INFORMATION
AFHP retains your personal information, as necessary, for business purposes and as required by government regulation. We dispose of records and delete information when our retention period(s) expire.
